note: I do not accept any form of insurance.  I am not enrolled as a Medicare or Medicaid provider. You can choose to find a provider that accepts your insurance. Services are private pay only. You can request a receipt to give to insurance. However all forms of reimbursement are to do with your insurance and should/ will not have any effect on the cost of my services. Myself or my company is not responsible for any form of reimbursement. All services should be paid in full at time of service.

-$50 cancellation fee will be charged if  client cancels within 24 hrs. of appointment.
