Options 3,4 can be used after option 1 or 2. If Sessions exceed hour limit for each month then normal rate of $20.6/15 minute session will be charged. Package bundle must be paid up front.

Sessions must be used within 30 days regardless of cancellations and holidays. No refunds. For New Clients only.

note: I do not accept any form of insurance. Services are private pay only. You can request a superbill to give to insurance. However all forms of reimbursement are to do with your insurance and should/ will not have any effect on the cost of my services. Myself or my company is not responsible for any form of reimbursement. All services should be paid in full at time of service.
-Any care updates are charged under the treatment price
-$50 cancellation fee will be charged if client cancels within 24 hrs. of appointment.
*Notice a 3% service charge will take place for all payments
** Price will be rounded to to full 15 minute charge if 8 mins and over. Price will be not adhere a 15 minute charge if under 8 mins.
